Submission Test

- I grant permission to NAMI San Diego and other associated organizations to the use my name, age, and image (photographs and/or videos) of which I submitted with this form.
- I consent to NAMI San Diego and other associated organizations’ use of my name, age, and image (photographs and/or videos) for use in their productions, on their websites, across their social media, or however they see fit.
- I hereby waive any right to inspect or approve the finished photographs or electronic matter that may include my name, age, and image (photographs and/or videos) for current or future use, whether that use is known or unknown to me.
- I hereby waive any right to the potential royalties or other compensation that may arise from the use of my submissions.
- I understand that my submission is not guaranteed to be shown during the event or in related material surrounding the event.
